A study analyzed 814 patients who had colorectal adenomas with high-grade dysplasia (HGD) removed, revealing significant risks of advanced neoplasia (AN) and colorectal cancer (CRC). Over 2000 person-years of follow-up showed AN and CRC incidence densities of 44.3 and 4.4 per 1000 person-years, respectively. Synchronous polyps and elevated baseline carcinoembryonic antigen (CEA) levels were key risk factors. However, surveillance colonoscopies markedly reduced AN and CRC incidences.
