Staged Surgery May Lead to Safer Outcomes in Colorectal Cancer With Liver Spread

For some patients with colorectal cancer that has spread to the liver (synchronous colorectal liver metastases, or sCRLM), surgery can remove both the bowel tumor and the liver tumors. Doctors may choose to do both operations at the same time (simultaneous resection) or in separate steps (staged resection).

A large international study of nearly 900 patients looked at which approach leads to better results. Researchers measured outcomes using something called textbook outcome in liver surgery (TOLS). This means the surgery went smoothly without major complications like infections, liver failure, bile leaks, readmissions, or death within 90 days.

The study found that:
– Overall, 63% of patients achieved a textbook outcome after surgery.
– Patients who had staged surgery were more likely to achieve good results than those who had both surgeries at once (66% vs. 59%).
– The chances of a smooth recovery dropped sharply for patients who needed both a high-risk bowel operation and a major liver operation at the same time.

These findings suggest that staging the surgeries may be safer, especially for patients requiring extensive operations. Doing the surgeries in steps may reduce complications and improve recovery.

Patients facing surgery for colorectal cancer with liver spread should discuss with their doctors whether a staged approach might offer the best balance between safety and effectiveness.
