Comprehensive Real-World Outcomes of Multimodal Treatment in Gallbladder Cancer A Prospective Cohort Study. DOI 10.1007s12029-026-01425-x

Gallbladder cancer is a serious disease that is especially common in parts of North India. Many patients are diagnosed late, when the cancer has already spread and treatment options are limited.This large study followed 1,500 newly diagnosed patients to understand how different treatments work in real-life settings. Doctors looked at three main treatment approaches: chemotherapy before surgery, immediate surgery, and chemotherapy for advanced cancer.The results showed a major challenge. More than 80% of patients already had metastatic disease when they first came to the hospital. For these patients, treatment usually focused on chemotherapy to control symptoms rather than cure the cancer.Among patients who received chemotherapy before surgery, most tumors continued to grow and only a small number were able to have curative surgery afterward.The best outcomes were seen in patients who were able to have surgery to remove the cancer. These patients had a 1-year survival rate of nearly 89%, which was much higher than those treated with chemotherapy alone.
For patients with metastatic disease receiving palliative chemotherapy, the average survival was about five months.
The study highlights an important message: surgery offers the best chance of survival in gallbladder cancer, but it is only possible when the disease is found early.
