Can Some Rectal Cancer Patients Skip Radiation Before Surgery?

For patients with locally advanced rectal cancer, treatment often includes chemotherapy, radiation, and surgery. While this approach is effective, radiation can sometimes cause long-term side effects. Doctors are now studying whether some patients may safely avoid radiation.

A new study looked at nearly 7,000 patients with stage II and III rectal cancer who received chemotherapy before surgery. Most patients had the standard combination of chemotherapy plus radiation (chemoradiotherapy), while a smaller group had chemotherapy alone.

The results showed that both groups had similar survival rates and similar chances of having no cancer left after surgery (pathologic complete response). In other words, skipping radiation did not reduce survival for selected patients.

The study also confirmed that the most important factors for survival were whether all the cancer could be removed during surgery and whether patients achieved a complete response, not whether radiation was included.

These findings suggest that for some rectal cancer patients, chemotherapy alone before surgery may be enough, helping them avoid the side effects of radiation. But this approach may not be right for everyone. Careful patient selection is key, and more studies are needed to confirm which patients can safely skip radiation.

Patients should talk to their oncologist about whether their cancer type and stage make them candidates for chemotherapy-only treatment.
