Chemoradiotherapy Before Surgery Improves Long-Term Survival in Low Rectal Cancer

For patients with low rectal cancer, treatment often includes a combination of chemotherapy, radiation, and surgery. Doctors continue to study the best timing for this combined approach—whether patients should receive chemoradiotherapy before surgery (neoadjuvant) or after surgery (adjuvant).

A large study from China followed more than 1,100 patients with stage II and III low rectal cancer treated between 2006 and 2021. Researchers compared long-term survival between the two strategies.

The results showed that for stage II patients, survival was similar whether treatment was given before or after surgery. But for stage III patients, the difference was striking. Patients who had chemoradiotherapy before surgery lived much longer and had fewer recurrences than those treated afterward.

– Five-year survival:
80% with before-surgery treatment vs. 61% with after-surgery treatment
– Ten-year survival: 78% vs. 45%
– Five-year disease-free survival: 70% vs. 48%

This means that giving chemotherapy and radiation before surgery nearly doubled the long-term survival rates in stage III low rectal cancer patients.

These findings support neoadjuvant treatment as the preferred approach for advanced low rectal cancer. It not only improves the chances of survival but also lowers the risk of the cancer coming back. Patients facing surgery for stage III low rectal cancer should ask their doctors whether this treatment plan is right for them.
