Background
This systematic review aims to evaluate the effectiveness and safety of 2D versus 3D laparoscopy for the treatment of colorectal cancer.
Methods
A comprehensive literature search was conducted through PubMed, Web of Science, and Embase from their inception to January 2024. The studies included in this meta-analysis investigated various outcomes of colorectal surgery. The results were expressed as odds ratios (ORs) or mean differences (MDs) with 95% confidence intervals (CIs). The protocol for this review was registered on PROSPERO (CRD42024504902).
Results
A total of 10 publications met the inclusion criteria. The findings indicate that the 3D laparoscopy group had:
- Intraoperative Blood Loss: Significantly lower blood loss (MD = -80.4, 95% CI = -141.8 to -18.9, P = 0.01).
- Operative Time: Reduced operative time (MD = -17.33, 95% CI = -29.15 to -5.51, P = 0.004).
- Postoperative Hospital Stay: Shorter hospital stays (MD = -0.43, 95% CI = -0.74 to -0.12, P = 0.006).
- Total Complications: Lower overall complication rate (OR = 0.94, 95% CI = 0.67 to 1.31, P = 0.71).
- Specific Complications: Reduced incidence of postoperative overall complications and anastomotic leakage.
There was no significant difference in the time of the first bowel movement, the incidence of anastomotic leakage, the number of dissected lymph nodes, and the number of harvested lymph nodes between the 2D and 3D groups.
Conclusions
The analysis suggests that 3D laparoscopy offers several benefits over 2D laparoscopy in colorectal cancer surgery, including reduced blood loss, shorter operative time, and decreased postoperative hospital stay. However, the advantage of 3D laparoscopy for radical colon cancer surgery, particularly in terms of the number of dissected lymph nodes, may not extend to rectal cancer surgery.
